At this time I am only offering telehealth services.

Areas of emphasis include life transitions, depression, anxiety, and other mood concerns, and LGBTQIA+.


Rates

  • Initial Telephone Screening (15 Minutes): No Charge

  • Initial Visit/Clinical Evaluation: $250

  • Individual Therapy: $200/hour

  • Written Report: $150/report

  • Letters/Form Signature: $50/letter or form

  • Consultation: $200/hour

  • Bounced Check Fee: $35/check

I offer a 15-minute initial telephone consultation, free of charge, to discuss your therapeutic needs, and to see if we are a good match to best address your concerns. Office visits and other services are charged as listed above and fees are collected at the time of service. Fees are payable by check, cash, debit card, or credit card, and are subject to change without notice.

Everbreeze Psychological Services does not offer a sliding fee scale.


Insurance

Everbreeze Psychological Services currently only accepts Blue Cross/Blue Shield (BC/BS). Please make sure I am an approved provider for your specific insurance plan.

It is the patient's/parents'/guardian's responsibility to ensure that I am covered by the patient's insurance plan.

Many insurance companies offer out-of-network benefits for patients seeking psychological services and will reimburse a percentage of fees charged. To find out if you have out-of-network benefits, please call your insurance company and ask them the following questions:

  • Do I have out-of-network mental health benefits?

  • How much does my plan cover for out-of-network providers?

  • How many sessions are covered, and under what time period?

  • Do I have an annual deductible, and has it been met?

  • Do I need authorization for outpatient mental health services?

  • What does my plan allow (“reasonable and customary”) for the following codes: 90791, 90837, and 90834?

  • How do I go about obtaining reimbursement?

  • What forms do I need to submit, where do I get them, and what information do I need to provide in order to be reimbursed?
